To receive your $5 mail in rebate, purchase any combination of two NeilMed^® Sinus Rinse^™ Kit or NasaFlo^® Neti Pot at your local Walmart store, submit original UPC Bar Code along with a copy of your receipt with your name, complete address, email and phone number to the below mentioned address. Please allow up to 30 days to receive the rebate check. Rebates do not include tax. Products must be purchased between July 25, 2011 to August 14, 2011 and submitted by August 31, 2011.
